This link seems to be team entry for the pre champs days. Is there a form for the actual Champs days or is it somewhere and I have missed it?

Oh wait. Now I see. I'm getting the team for the year competition confused with Champs teams. My bad.

Thank you Sharon and Shirlene for posting to my question.  I still have a few more questions.....does each team member send in a separate entry?  How do you know who the team members are?  Is there a place to write a team name, do you you even need a team name?  Is it retroactive to January 1st, 2016?  sorry, but never having done this before, I don't want to screw it up.

Yes, each team member must fill out the form.  It is retroactive to January.
